Terms and Conditions Agreement

By booking any session, I agree to the following terms of understanding:

Competency and Acknowledgment

I confirm that I am a fully competent adult, 18 years or older.

Indemnity and Release of Liability

I indemnify and hold harmless Brittany (Bunny) Urick, "Practitioner," and Prama Soma from any causes of action arising from my presence at the premises, including but not limited to trip-and-fall claims, personal injury, pain, suffering, emotional or psychological harm, property damage, and/or claims arising from homeowner negligence or accessibility issues. I assume full responsibility and risk for all my activities, persons, and property while on my premises.

Waiver of Liability

I release, waive, discharge, and covenant not to sue Brittany "Bunny" Urick, Prama Soma, or any of its affiliates or agents (“Practitioner”) from all liability to myself, my heirs, assigns, or next of kin for any loss or damage to body, mind, or property, whether accidental, unintentional, or not caused by Practitioner, solely or contributorily.

Informed Consent

I acknowledge that by booking with Prama Soma, and signing this document, I am waiving certain rights that I or my heirs may have against the Practitioner.

Use of Technology

I understand and accept the risks of using technology, including email, text, phone, and video conferencing platforms like Google Meet and Zoom. If I choose to record my sessions, I consent to my image and voice being recorded.

Non-Medical Nature of Services

I understand that all services provided by my Practitioner are not a substitute for professional mental health care or medical care and are not intended to diagnose, treat, or cure any mental health or medical conditions.

Bodywork and Coaching Policy

I acknowledge that Bodywork, Reiki, and Coaching services are entirely therapeutic and non-sexual in nature. Any inappropriate behavior will result in immediate session termination, refusal of future services, and a police report.

Personal Responsibility and Health Updates

I agree to inform my Practitioner of any changes in my health and medical condition and understand that I am responsible for doing so. I also agree to reschedule my appointment if I have symptoms of illness or if I’ve been in contact with someone ill.

Acknowledgment of illness risks

I understand that bodywork involves close physical contact and acknowledge the elevated risk of disease transmission. I voluntarily accept these risks and release the Practitioner from any related claims.

Financial Responsibility and Payment Terms

I am responsible for all charges for services rendered. I understand there are no exchanges or refunds for services rendered, pre-paid packages, or memberships.

Cancellation, Rescheduling, and No-Show Policy

I agree to provide a valid credit card to be held on file for no-shows or late cancellations. I authorize Brittany "Bunny" Urick to keep my designated Credit Card on file for any membership charges, agreed-upon payment plans, and non-refundable deposits. 

I understand that if I fail to change or cancel an appointment with at least 48 hours' notice, I forfeit my non-refundable 50% deposit for my session, and I will forfeit that session if it is part of a package or membership. 

I understand that if I fail to attend or reschedule a complimentary Welcome Call with 48 hours' notice, I will forfeit a second free call. 

Wellbeing Program Participation

I agree to participate fully in my well-being program and make sound choices regarding the plan developed with my practitioner. I will let my Practitioner know if I feel my well-being is compromised at any time.

Coaching and Safety Expectations

I expect my Practitioner to provide safe and effective coaching using their skills and knowledge, and I agree to communicate openly about any concerns I have.

Marketing and Communication Consent

I agree to be contacted by my Practitioner via email, text, and phone for appointment reminders and marketing purposes.

Expiration of Sessions and Packages

I understand that sessions in packages or purchased with gift certificates will expire after one year or the agreed-upon time frame from the date of purchase.

Use of Testimonials

I authorize my Practitioner to use any testimonials or feedback I provide for promotional purposes. I waive any right to inspect or approve the finished product and release the Practitioner from any claims related to using my testimonial.

Karma Clause

The practitioner agrees to treat the client with love, care, and good vibes. In turn, the Client agrees to treat the Practitioner with consideration and respect. Should either party fail to uphold this agreement, it won’t result in a material breach of this contract; the breaching party will bear any ensuing lousy karma.

General Release of Claims

I am hereby released, waiving, acquitting, and forever discharging my Practitioner, its agents, employees, and successors from any claims or damages that come from this agreement or the advice given by the Practitioner. This agreement binds my heirs, executors, and assigns.
